How to create custom form templates

Create custom intake forms and charting templates using Vera or the manual form builder.

TL;DR: Create custom forms with Vera (share a PDF and Vera builds it) or manually through your organization's Custom Form Templates settings. Use templates for intake forms or clinical documentation.

Overview

Village allows you to create custom form templates for intake and clinical documentation. You can build templates with Vera's help or create them manually using the form builder.

Option 1: Create a template with Vera

  1. Open a chat with Vera.

  2. Tell Vera what template you want to create.

  3. Share the PDF template you want to replicate.

  4. Vera will build the form template for you.

  5. Review and edit the template if needed (see manual steps below).

Option 2: Create a template manually

  1. Click on your Organization in Village.

  2. Go to the Custom Form Templates tab.

  3. Choose either Charting Templates (for clinical documentation) or Intake Templates (for patient intake forms).

  4. Click Create New Template.

  5. Build your form using the template editor.

  6. Save when finished.

Editing templates

Whether Vera created the template or you built it manually, you can edit any template from the Custom Form Templates screen. This is where you finalize formatting, add or remove fields, and make adjustments.

  • Intake Templates β€” for new patient intake forms

Can Vera recreate a PDF form I already use?
Yes. Share the PDF with Vera and Vera will build a digital version as a template in Village.

Who can create and edit templates?
Template creation and editing is typically available to organization admins. Check with your org admin if you don't see these options.

Can I have multiple templates?
Yes. You can create as many charting and intake templates as your organization needs.
